IF YOU ARE 65 OR BETTER, YOU ARE ELIGIBLE TO VOTE BY MAIL
SEND IN YOUR BALLOT REQUEST FORM TODAY

Texas Election Law requires one of the following identification numbers to be included in your application: Texas Driver’s License, Texas Personal Identification Number OR Texas Election Identification Certificate Number. If you do not have one of these, give the last 4 digits of your Social Security Number.

Step 1: Fill in your preferred ID# in the application.

Step 2: Sign the attached mail ballot request form (one per voter).

Step 3: Place in an envelope, address to:
